Engine failure, brush fire: United Airlines flight returns to Dulles shortly after takeoff

A United Airlines flight headed to Tokyo had to return to Dulles International Airport shortly after takeoff due to engine failure, with no injuries reported among the 275 passengers and 15 crew members. A piece of the engine cover separated, causing a brush fire on the ground, which has since been extinguished. The FAA is investigating the incident.
